1. TAKE SHADE AND HARP (SHADE HOLDER OFF) THEN TAG THE LAMP WITH NAME ADDRESS AND PHONE NUMBER AND TYPE OF SOCKET AND COLOR OF CORD NEEDED.

2.WRAP THE LAMP IN PROTECTIVE CLOTH OR BUBBLE WRAP

3. MAIL OR SHIP VIA UPS OR FEDERAL EXPRESS TO: P.O. BOX 807; 3567 S.
